GP Management Plans
GP management plans form an important part of any chronic disease management. GP Management plans enable us to reduce the costs of accessing allied health providers. At Stirling GP we take pride in developing tailored health plans to suit your needs. Enquire with our staff today about booking your management plan.

Mental Health Plans
Mental Health Plans give us the opportunity to work collaboratively with you and a psychologist to best manage your mental health. Under the current COVID scheme up to 10 visits to a psychologist are subsidized by the government.
Please allow minimum 30 minutes for your MHCP (advise staff of booking)
45 – 49-year-old Health Check
We all are at risk of sometimes overlooking our health needs. The Government encourages a 45 -49 health check. During your health check we will; Check your blood pressure, perform an ECG, Review your blood tests and ensure that we discuss your health plans moving forwards. This health check is an excellent way to open the door to further health discussions and plan moving forwards.
Please allow 45min for your 45 – 49 health check (advise staff of booking)
75+ Health Assessment
Preventative health is important to us! At your 75+ health assessment we will review your medical history to date, ensure that all of your health screening (FOBT, Mammograms, Prostate etc) is up to date. We will perform a comprehensive blood review and ECG of your heart. The 75+ health assessment gives us the opportunity to not only review your health to date, but to plan for better health moving forwards.

Home Medication Review
Too many medications? Unsure why the medications are necessary? The Home Medication Review allows a specialist pharmacist to review your medications and make additional recommendations.
The Home Medication Review is an excellent option to help consolidate your medications – stopping unnecessary medications or updating medications to suit new evidence or your personal health needs.
Please advise staff of your request for a HMR
Allow 1 -2 hours for a home visit from pharmacist

SA pathology provides pathology to the public and private health systems which ensures the seamless sharing of your results between hospital and general practice.
