New Patient Registration

New patients you can save time by printing out the New Patient Registration Form. Complete this form prior to your appointment at Stirling GP and hand it in to our friendly reception staff when you arrive.

Transfer of Medical Records

To ensure a seamless transition of medical records we kindly request that you complete a Transfer of Medical Records form, available from reception. We will request that your previous clinic’s clinical records are transferred to ours in a timely manner. As a patient you have a right to your medical records and our team will happily assist you with transferring your records either to or from our clinic!
